In the US or Canada?

Once we receive your info, we'll email you for the following:

• A completed 'Wholesale Account Information Form' in either PDF or Microsoft Excel format
• Your business license and/or resale certificate
• Jpegs of your store front and store interior (email only) 

Note: We will contact you for this information after we review your form submission. You don't need to do anything yet, but keep in mind we will need these documents to setup your new wholesale accounts.
